PICC Contact Information

The cost of membership is $20.00.

If you are currently an active NYS LEAH member in another NYS LEAH Chapter, membership is FREE, but you still have to fill out the membership form and mail it in.

If you do NOT have email access and/or printing capability and need your chapter documents and newsletters mailed to you, you are required to pay an annual administration fee to cover the cost of copies, envelopes, and postage. Please refer to the membership form for the various membership levels available to those without email/internet and/or printing capability.

If you wish to join PICC LEAH, please print the following form, fill it out, and mail it in.
